This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A complaint inspection was initiated on November 10, 2020 and concluded on December 10, 2020. A complaint incident was received by the department regarding allegations in the areas of 22VAC40-111-(7), 22VAC40-191-(BC) and22VAC40-111-(8). The provider was contacted by telephone to conduct the investigation.
The evidence gathered during the investigation supported the allegation of non-compliance with standards or law and violations were issued. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.

Standard #: 22VAC40-111-240-A Complaint related: Yes Description: Based on documentation from a local agency and photograph, the family day home failed to be maintained in a clean and safe condition.
1. On October 30, 2020, local agency observed black mold on the ceilings of the bathrooms in the family day home.
One of the bathrooms is used for the children in care.
2. The provider confirmed there was black mold on the both bathroom ceilings.Plan of Correction: ServPro completed mold remediation and cleaned and restored the areas that were affected. Contract and certification was given to the licensing inspector.
