54.1 Provider must be MAT certified to administer prescription medication.

Technical Assistance:
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. Licensing Inspector stressed mask wearing for parents and caregiver at drop-off and pick-up.

Consultation was given on allergy action plan, the need for MAT to administer emergency medication and model forms (or all information is covered in the FDH paperwork).

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on 11/30/2020 and concluded on 12/11/2020. The Provider was contacted by email to initiate the inspection. There were six children with one caregiver for sixteen points which is within ratio for supervision. Reviewed nutrition, children, provider and household member?s records, trainings and emergency preparation. No medication was reported to be on-site the day of the inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-111-880
Description: Based on provider statement, the family day home meal plan does not follow the most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA).
Evidence:
The provider stated that she serves juice at lunch and not milk.

Plan of Correction: Milk is always on premises and offered to parents but go on parents guidance for each child.

Standard #: 22VAC40-111-60-A
Description: Based on provider's statement during the remote inspection, 1 of 2 children's records was incomplete.
Evidence:
Child #1 has documented allergies and the "actions to take in case of an emergency" can not be completed as there is no medication on-site, as listed, to administer during an allergen exposure.

Plan of Correction: Have emailed 2 individuals for MAT training-waiting for a response-date to be completed in process.

Standard #: 22VAC40-111-70-A
Description: Based on the provider's statement during the remote inspection, a child does not have written documentation that the parents were given all required information.
Evidence:
Child #2 is missing signed documentation of the parent's acknowledgement of the "Information For Parents" form as well as the "Insurance Liability Declaration" form.

Plan of Correction: All clients will fill out complete paperwork to update for all missing, changed or COVID situations after my week off 12/26-1/3

Standard #: 22VAC40-111-680-B
Description: Based on the provider 's statement, the caregiver does not ensure that children's hands are washed with liquid soap and warm running water after diapering.
Evidence:
When the Licensing Inspector asked about hand washing the provider stated she did not wash children's hands after diapering.

Plan of Correction: Had missed this in the regulations and has never been bought to my attention.
