Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. An unannounced monitoring inspection was initiated on 11/13/2020, and concluded on 12/04/2020, approximately from 9:17 am to 10:00 am. There was 1 child supervised by 1 direct care staff. The inspector emailed the provider a list of items required to complete the inspection. The physical plant, 1 staff record, 3 household records, 1 child?s record, children activities, children's activities, emergency procedures, drills and supplies were inspected. During the inspection, child was observed in art activity. The interactions between the children and caregivers were seen to be positive and constructive in nature. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. Description:	Based on records review, the provider did not maintain for each caregiver and adult household member a current Report of Tuberculosis Screening (TB) form every two years from the date of the first screening or more frequently as recommended by a physician or the local health department. Evidence: 1. Staff #1 TB expired on 10/05/2018 2. Household member #1 TB expired on 10/16/2018 3. Household member #2 TB expired on 8/30/2016 4. Household member #3 TB expired on 10/16/2018
