Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 12/15/2020 and concluded on 12/21/2020. The provider was contacted by telephone to initiate the inspection. There were 3 children present and 2 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 4 staff / household members records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. Standard #:	22VAC40-111-100-A
Description:	Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. evidence: 1) The records for Child #1 (start date 09/14/2020) and Child #2 (start date 01/29/2018) did not contain documentation of a physical examination.
Plan of Correction:	The provider shall contact the parents and request the information.
Standard #:	22VAC40-111-60-B
Description:	Based on record review, children's files did not contain all required information. Evidence: 1) The record for Child #2 did not contain the telephone number of each custodial parent's place of employment.

Standard #:	22VAC40-111-180-A
Description:	Based on record review, the provider did not obtain a current Report of Tuberculosis (Tb) Screening form, every two years, for each caregiver and adult household member. Evidence: 1) The record for Staff #2 and Household member #2 did not contain current Tb screening results. The most recent screening forms in the files were dated 09/11/2017 and 10/21/2017 respectively.
