Technical Assistance:
The provider recently moved to a new home location. This inspection included a virtual inspection of the new home and child care area in order to modify the address on the license. The new home location will again be fully inspected once the Inspector is permitted to return to on-site inspections.

Discussed the use of a portable heater in rooms not used for child care.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated and concluded on 1/5/2021. The provider was contacted by telephone to initiate the inspection. There were 4 children present with the provider. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-111-920
Description: Based on observations during this virtual inspection, it was determined that the posted menu was not fully completed as required.
Evidence:
The provider has not dated the posted menu to reflect the dates of the current one week period.

Plan of Correction: The menu will be dated.

Standard #: 22VAC40-111-100-A
Description: Based on a review of children's health records, it was determined that the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to one child's attendance or within 30 days after the first day of attendance.
Evidence:
The provider does not have a completed physical exam report in the file for child B. This child has been enrolled for more than 30 days.

Plan of Correction: The child is going to the doctor today. I will ask the parent to have the physical form completed.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that two records do not contain all required information.
Evidence:
Child record A does not include the father's work phone number.
Child record B does not include the address for each emergency contact person.

Plan of Correction: I will ask both parents for the information.

Standard #: 22VAC40-111-240-A
Description: Based on observations during this virtual inspection, it was determined that areas and furnishings of the family day home, inside and outside, have not been maintained in a clean, safe, and operable condition.
Evidence:
The Inspector observed a pile of coats and shoes on the stairs leading from the basement child care area to the upstairs level of the home. These stairs would be used an emergency exit route. The items piled on the stairs pose a tripping hazard and a safety hazard in the event that route is needed for an emergency exit.

Plan of Correction: I will move the items immediately.

Standard #: 22VAC40-111-250-C
Description: Based on observations during this virtual inspection, it was determined that the provider did not remove all crib toys hung from the side with strings or cords before one child turned five months of age.
Evidence:
The Inspector observed a crib mobile toy tied to the side bars of one child's crib. This child is age 11 months.

Plan of Correction: The toy will be removed.
