Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on January 07, 2021 and concluded on January 07, 2021. The provider or in-charge person was contacted by telephone to initiate the inspection. There were nine children present and three staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed two children?s records, two household members and two staff records submitted by the facility to ensure documentation was complete.

The information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word "corrected" is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Standard #: 63.2-1721.1-A
Description: Based on record review and interview, the provider did not ensure that every adult living in such family day home shall undergo a background check in accordance with subsection B prior to issuance of a license and every five years thereafter.

Evidence:
1. The central registry check for Household Member #1 was last dated 12/22/2015.
2. The sworn statement for Household Member #1 was last dated 11/13/2015.
3. The provider verified that the dates of the central registry check and the sworn statement.
