Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A complaint inspection was initiated on 07/13/2020 and concluded on 07/16/2020. A complaint incident was received by the department on 03/16/2020 regarding allegations in the areas of administration and care of children. The provider or in-charge person was contacted by telephone to conduct the investigation. The inspector emailed the provider a list of documentation required to complete the investigation.

Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word "corrected" is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

The evidence gathered during the investigation supported the allegation of non-compliance with standards or law and violations were issued. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.

1. Provider failed to notify parents the percentage of time per week persons other than provider would be caring for the children.
2. Provider verified that information was not provided to the parents.
