Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on 1/14/2021 and concluded on 1/15/2021. The provider was contacted by telephone to initiate the inspection. There were 3 children present (11 points) and 1 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 1 staff record submitted by the facility to ensure documentation was complete. There are no new household members or staff and all required backgrounds are current. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-111-60-B
Description: Based on record review, 1 of 2 children's records did not contain all required information.

Evidence:
1.The record for Child #2 did not contain the telephone number of 1 of 2 designated persons to contact in case of an emergency if a parent cannot be reached.
2. The phone number provided for the emergency contact person was the parent's phone number.

Plan of Correction: I am going to ask the parents for another phone number for the emergency contact.

Standard #: 22VAC40-111-70-A
Description: Based on record review, parents were not provided in writing with all required information.

Evidence:
1. 2 of 2 children's records did not contain documentation that the parents received written provisions of the family day home's emergency preparedness and response plan.

Plan of Correction: I will give the parents the forms today.
