Technical Assistance:
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Consultation was given on model forms, subsidy contact information to dis-enroll and health department contact information.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on 01/18/2021 and concluded on 01/21/2021. The Provider was contacted by cell phone to initiate the inspection. There were ten children with two caregivers for twenty seven points which is within ratio for supervision. Reviewed nutrition, children, caregiver and household member?s records, trainings and emergency preparation. No medication was reported to be on-site the day of the inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and a violation was documented on the violation notice issued to the facility

Standard #: 22VAC40-111-70-B
Description: Based on provider statement, 2 of 2 children's records reviewed did not have the parent signature to acknowledge receipt of the information.
Evidence:
Child #1 and #2 were incomplete as the parent signature was not obtained (the form was not complete)

Plan of Correction: The provider will reprint the second page of the Information for Parent form and have each family sign and date the form.
