Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on January 21, 2021and concluded on January 21, 2021. The provider was contacted by telephone to initiate the inspection. There were six (10pts) children present and two staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed two children?s records, three household member's records and two staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records did not contain required information.
Evidence:
1. Child #2's record did not contain a second emergency contact address and phone number.

Plan of Correction: Provider will obtain an address and a phone number for the listed name as a second emergency contact and document the record.

Standard #: 22VAC40-111-180-A
Description: Based on record review, the provider did not obtain for each caregiver and adult household member a subsequent tuberculosis screening.
Evidence:
1. Staff #1's record contained an expired tuberculosis screening form. The screening was dated 12/18/2018.
2. Household Member #2's record contained an expired tuberculosis screening form. The screening was dated 12/18/2018.

Plan of Correction: The provider will have a Tuberculosis Screening Form completed by a physician.

Standard #: 22VAC40-111-210-A
Description: Based on record review, caregiver did not obtain 16 clock hours of training annually in areas relevant to their job responsibilities.
Evidence:
1. Staff #1's record contained documentation of 8.75 hours of annual training.
2. Staff #2's record did not contain documentation of annual training. Staff #1 stated that no annual training was completed by Staff #2.

Plan of Correction: Staff #1 will complete 7.25 hours of on line training to meet the 16 hour requirement. Staff #2 will complete 16 hours of on line training.
