Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 01/19/2020 and concluded on 01/20/2021. The director/provider or in-charge person was contacted by telephone to initiate the inspection. There were 10 children present and 4 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-111-170-A
Description: Based on record review, the provider did not obtain from each caregiver at the time of hire a current Report of Tuberculosis (Tb) Screening form.
Evidence:
1) The record for Staff #1 (start date 11/09/2020) did not contain a Tb screening result.

Plan of Correction: Will request Staff #1 to provide a Tb screening.

Standard #: 22VAC40-111-210-A
Description: Based on documentation review, caregivers did not obtain 16 clock hours of training annually.
evidence:
1) Documentation provided for Staff #2 confirmed only 9.5 hours of training during the 2020 year period.

Plan of Correction: Moving forward, provider shall ensure all caregivers complete a minimum of 16 hours of training annually.

Standard #: 22VAC40-111-320-G
Description: Based on interview with the provider, fireplaces and associated chimneys were not inspected annually by a knowledgeable inspector to verify that the devices are properly installed, maintained, and cleaned as needed.
Evidence:
1) On the date of inspection, the provider stated that the fireplace has not been inspected.

Plan of Correction: I will get the fireplace inspected.
