Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated and concluded on 1/28/2021. The provider was contacted by telephone to initiate the inspection. There were (3) children present and (1) caregiver. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed (2) children?s records and (2) staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-111-800-B
Description: Based on interview, the provider did not review the emergency plan at least annually and document in writing each review and update to the emergency plan.
Evidence:
1. The provider stated the last review of the emergency plan was dated 3/20/2019.

Plan of Correction: I will review it today.

Standard #: 22VAC40-111-180-A
Description: Based on interview, the provider did not obtain updated tuberculosis (TB) test or screenings for each caregiver or household member every two years.
Evidence:
1. Caregiver #1 had a TB test or screening dated 6/26/2018. The provider stated she did not obtain an updated TB test or screening.
2. Household member #2 had a TB test or screening dated 6/29/2018. The provider stated she did not obtain an updated TB test or screening..

Plan of Correction: Will be done within 2 weeks.

Standard #: 22VAC40-191-40-D-5-a
Description: Based on interview with the provider, the provider did not obtain a central registry report within 30 days of a 14-year-old person in the home becoming 14 years old.
Evidence:
1. The provider stated she did not have the central registry report for the household member. She stated it was sent last year, but did not receive it back.

Plan of Correction: Will be done within 2 weeks.
