Technical Assistance:
Provided consultation on:
1) Background checks - household member who are age 14-18 are required to have documentation of a search of the central registry on file.
2) Children under two years of age shall not use pillows or filled comforters. No soft bedding of any kind shall be used under or around infants including but not limited to: pillows, quilts, comforters, sheepskins, or stuffed toys.
3) Discussed attending Pre-Licensure Orientation for Family Day Homes ? Phase 2. In this training a licensing inspector will focus on the Standards for Licensed Family Day Homes and answer questions you may have. The Phase 2 trainings are held in the regional licensing offices. You will need to call the office that serves your area and sign up for a class.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was conducted on 1/26/21 and 1/28/21 with an exit interview on 1/28/21. The provider was contacted by telephone and a virtual inspection was conducted. There were five children present, ranging from 11 months to three years of age, for a total of 13 points. The Inspector reviewed two children's records, one caregiver record, the emergency drill log, hand washing procedures, naptime, enhanced cleaning and sanitizing of toys and surfaces, meals and snacks, and daily indoor and outdoor activities. The information gathered during the inspection determined non-compliance with standards that are documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-111-920
Description: Based on review of documentation, the menu was not dated as required. Evidence: the posted menu was not current as it was dated: 1/17-1/23/2021.

Plan of Correction: Provider corrected the date during the inspection. Future menus will be dated accurately.

Standard #: 22VAC40-111-100-A
Description: Based on a review of two children's records, the provider did not obtain documentation that each child has received a physical examination by or under the direction of a physician prior to the child's first attendance or within one month after attendance. Evidence: the record for child #1 (date of first attendance: 12/7/20) did not have documentation of a physical examination.

Plan of Correction: Provider stated that she will ask the parents for documentation of a physical examination.

Standard #: 22VAC40-111-60-A
Description: Based on review of two children's records, it was determined that not all records contained all required information. Evidence: the record for child #1 did not have the name, office address and telephone number of the child's physician. The record for child #2 did not have the name, address and phone number for a second emergency contact.

Plan of Correction: Provider stated that she will ask the parents for the missing information.

Standard #: 22VAC40-111-530-G
Description: Based on an interview with the provider, children under two years of age use pillows during naptime. Evidence: the provider stated that child #1, age 11 months (date of birth: 1/24/20) who sleeps in a crib, and child #2, age 23 months (date of birth: 2/2/19) who sleeps on a rest mat, use pillows during naptime.

Plan of Correction: Provider stated she will not use pillows any longer during naptime.
