Technical Assistance:
The Licensing Inspector stated to the provider that his subsidy status online was listed as "No." It was later clarified that Mr. Smith is still listed twice within the system and that his vendor information has yet to switch over within the VDSS system to solely licensing. The Provider was given the phone number of the Vendor Subsidy Manager and was encouraged to make contact on his end. Licensing was told that subsidy should be reaching out to the Provider as well.

A black copy of the Virginia CPS check, search of the central registry was sent to the Family Day Home for Household Member #2.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A non-mandated, monitoring inspection (NMI) was initiated and concluded on 01/25/2021. This inspection was a follow-up to the initial inspection and has taken place at the 2 month mark of the conditional licensing period. The provider was contacted by telephone to initiate the inspection. There were (3) children, plus (1) resident child present, (9) points, with (2) caregivers. This inspection reviewed those violations cited during the initial inspection and any standard that remained not determined. The Inspector reviewed (3) children?s records, (3) household files, and (2) staff records submitted by the Family Day Home (FDH) to ensure documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and were documented on the violation notice issued to the FDH.

Violations:
Standard #: 22VAC40-111-760-A
Description: Based on interview, the first aid kit did not include all of the required items.

Evidence -
1. The Provider stated (during an interview conducted on the date of inspection - 01/28/2021) that the first aid kit is still missing a set of tweezers.
2. The Provider stated that he went to Walmart, but were not in stock.

Plan of Correction: Tweezers will be purchased from a local pharmacy.

Standard #: 22VAC40-111-790
Description: Based on interview, not all of the required information was posted.

Evidence -
1. The Provider stated (during an interview conducted on the date of inspection - 01/28/2021) that the information of a responsible person for emergency backup care as required in 22 VAC 40-111-800 A 3 was not posted.
2. The Emergency Preparedness and Response Plan (EP&RP) completed by the Provider has yet to be printed out. The emergency back-up person's information was listed there.

Plan of Correction: The EP&RP shall be printed out and posted next to the other posted emergency phone numbers.

Standard #: 22VAC40-191-40-D-5-a
Description: Based on interview, 1 of 3 household member files indicated that a person living in the home of a licensed Family Day Home (FDH) provider did not obtain a Child Protective Services central registry check (CPS check) within 30 days of a person in the home becoming 14 years old.

Evidence -
1. The Provider confirmed (during an interview conducted on the date of inspection - 01/28/2021) that a CPS check for Household Member #2 was not conducted within 30 days of becoming 14 years of age.
2. The Provider stated that he confused this form with the fingerprint check.

Plan of Correction: This request shall be completed and sent for the appropriate finding.
