Technical Assistance:
The Licensing Inspector has reviewed with the Provider COVID-19 Essential Guidance for Child Care programs.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated and concluded on 01/29/2021. The Provider was contacted by telephone to initiate the inspection. There were (10) children present, (23) points, with (3) caregivers. The inspector emailed the Provider a list of items required to complete the inspection. The Inspector reviewed (2) children?s records and (4) staff records submitted by the Family Day Home (FDH) to ensure documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and were documented on the violation notice issued to the FDH.

Violations:
Standard #: 22VAC40-111-140-B-3
Description: Based on review, the Provider did not have a current certification in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care.

Evidence -
1. On the date of inspection (01/29/2021), the CPR certification on site had an expiration date of 05/19/2020.
2. This was confirmed by the Provider on the date of inspection.

Plan of Correction: A class to certify the Provider in CPR shall be scheduled and completed.

Standard #: 22VAC40-111-140-B-4
Description: Based on review, the Provider did not have a current certification in First Aid.

Evidence -
1. On the date of inspection (01/29/2021), the First Aid certification on site had an expiration date of 05/19/2020.
2. This was confirmed by the Provider on the date of inspection.

Plan of Correction: A class to certify the Provider in First Aid shall be scheduled and completed.

Standard #: 22VAC40-111-180-A
Description: Based on review, 3 of 4 staff files indicated that the Provider did not obtain for each caregiver a current Report of Tuberculosis Screening (TB) form, in accordance with the requirements in 22 VAC 40-111-170, every two years from the date of the most current documentation on file at the Family Day Home (FDH).

Evidence -
1. On the date of inspection (01/29/2021), the following staff had TB documentation that had expired: Staff #1 (dated - 09/16/2018), Staff #2 (dated - 11/19/2018), and Staff #4 (dated - 09/26/2018).
2. The Provider confirmed these findings on the date of inspection.

Plan of Correction: Updated TB documentation shall be obtained and placed on file for all applicable staff.
