Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 01/21/2021 and concluded on 01/22/2021. The provider was contacted by telephone to initiate the inspection. There were 12 children present and 2 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff/household records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-111-780-B
Description: Based on record review, the provider did not review with the parent, annually, the emergency contact information to ensure the information is correct.
Evidence:
1) The most recent emergency contact information review documented in the records for Child #1 and Child #3 were dated 10/2019.

Plan of Correction: I will review these emergency contacts with their parents annually.

Standard #: 22VAC40-111-920
Description: Based on documentation review and interview with the provider, the menu did not contain all required information.
Evidence:
1) The menu provided to the licensing inspector for review was not dated as required.
2) The provider stated to the licensing inspector that the lunch given to the children that day consisted of lentils with chicken, carrots, crackers and milk. The menu provided listed today's lunch as serving chicken breast, mixed vegetables, sweet potatoes, wheat bread and milk. The substitutions were not listed on the menu.

Plan of Correction: I will make sure the menu is dated accordingly going forward, Substitutions will also be updated.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records did not contain all required information.
Evidence:
1) The records for Child #1 and Child #3 did not contain the policy number of the child's medical insurance.

Plan of Correction: Policy number for Children #7 and #3 will be requested from parents.

Standard #: 22VAC40-111-210-A
Description: Based on documentation review, caregivers did not obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.
Evidence:
1) The certificates submitted for review for Staff #1 and Staff #3 documented a total of 13 hours and 3 hours of training respectively, for the year 2020.

Plan of Correction: I will ensure to have 16 hours of training annually, going forward.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation, the provider did not ensure that the findings of the most recent inspection of the facility were posted.
Evidence:
1) On the date of inspection, the Inspection Report posted was dated 08/07/2019.

Plan of Correction: I will ensure that the most recent inspections findings are posted.
