This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 02/01/2021 and concluded on 02/02/2021. The provider was contacted by telephone to initiate the inspection. There were 12 children present and 3 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 3 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. Standard #: 22VAC40-111-70-A Description: Based on record review, the provider did not provide parents in writing with all required information.
Evidence:
1) The record for Child #1 did not contain a signature acknowledging having received the Provisions of the family day home's emergency preparedness and response plan. The record for Child #2 did not contain a signature acknowledging having received the policies for the administration of medications.Plan of Correction: I will ask the parents for their signature.
