Comments:
On 1/20/21 a virtual renewal inspection was completed. Due to the COVID 19 pandemic the inspector was not on site. This inspection was completed via mobile video application as well as email and phone correspondence. During inspection there was 1 child present, observed playing freely and under the direct supervision of the provider. The facility and emergency supplies and procedures were inspection. Staff and Household members records were reviewed. Violation are indicated in the violation notice.

Standard #: 22VAC40-111-60-B
Description: Based on review, it was determined that all required information was not maintained.

Evidence: During inspection Child A was missing: Parent work place and phone number as well as address for the emergency pickups.
Child B was missing 2 of 2 emergency contacts.

Plan of Correction: I will get the information from their parents this week.
