Technical Assistance:
Provider's TB Screening will expire in the week and it is two years for each screening.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on February 5, 2021 and concluded on February 5, 2021. The Provider was contacted by telephone to initiate the inspection. There were 7 children present and 2 staff for a total of 20 points. The Provider was able to renew the MAT certification and had menu and other items posted for the parents. The Inspector reviewed children?s records and staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance with applicable standards or law and the violation was documented on the violation notice issued to the facility or the program. Please call with questions 703-479-4679

Violations:
Standard #: 22VAC40-111-180-A
Description: Based on record review it was determined the Provider did not have a family member obtain a current Report of Tuberculosis Screening form, in accordance with the requirements in 22 VAC 40-111-170, every two years from the date of the first screening.
Evidence:
1. Household members TB Screening was conducted over 2 and half years ago.
2. The Provider did not realize it had expired.

Plan of Correction: I will send the family member to obtain a new Screening.
