Technical Assistance:

An out of state search of the sex offender registry must be completed for all staff/adult household residents (Family Day Homes) who have resided outside of the state of Virginia within the past 5 years. In addition, an out of state criminal history record search must be completed for staff/adult household residents who have resided in any non-NFF states within the past 5 years. This requirement also applies to staff who currently reside outside of the state of Virginia.

Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated and concluded on 2/5/2021. The provider was contacted by telephone to initiate the inspection. There were 7 children present and 3 caregiver staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 3 staff/adult household member records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Description:	Based on a review of children's records, it was determined that one record does not contain all required information. Evidence: Child record A does not include documentation of the annual review of the child's emergency contact information.
Plan of Correction:	I will ask the parent to review the registration form.
Standard #:	22VAC40-111-180-A
Description:	Based on a review of caregiver health records, it was determined that three TB Health Screenings have not been repeated every 2 years. Evidence: The following TB Health Screenings have expired: Provider (expired 6/23/20); Assistant Caregiver A (expired 11/20/20); Adult Household Member A (expired 6/23/20)
Plan of Correction:	We will all get new TB screenings completed. Due to COVID, we will need to make appointments when available.
