Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on 02/03/2021 and concluded on 02/08/2021. The provider was contacted by telephone to initiate the inspection. There were 3 children present with the provider. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 3 staff and household records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-111-780-B
Description: Based on record review, the provider did not review with the parent the emergency contact information required to ensure the information is correct, and obtain the parent's signed acknowledgment of the review.
Evidence:
1) The most recent review of the emergency information in the record for Child #1 was dated 09/17/2019.

Plan of Correction: I will contact the parents to provide the review of the emergency information.

Standard #: 22VAC40-111-90-A
Description: Based on record review and interview with the provider, the provider did not obtain docume ntaion that the child has been adequately immunized.
Evidence:
1) The record for Child #2 did not contain documentation to confirm that the child has been adequately immunized.
2) The provider could not locate the documentation in Child #2's file.

Plan of Correction: I will contact the mother to request a copy of the immunization record.

Standard #: 22VAC40-111-180-A
Description: Based on record review and interview with the provider, the provider did not obtain a current Report of Tuberculosis (Tb) screening form, every two years.
evidence:
1) The most recent Tb screening report in the record for Household Member #1 was dated 03/07/2018.
2) The provider stated that she did not realize it had expired.

Plan of Correction: I will schedule an appointment.

Standard #: 22VAC40-111-210-A
Description: Based on documentation review, caregivers did not obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.
Evidence:
1) The inspector asked the provider to provide documentation of the training hours completed by caregivers for the year 2020. From the documentation provided, Staff #1 completed only 7.5 hours and Staff #2 did not complete any training hours in the year 2020.

Plan of Correction: The provider will ensure that annual training hours are completed as required.
