Comments:

This Inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 02/08/2021 and concluded on 02/10/2021. The provider was contacted by phone to initiate the inspection. There were 2 children present and 1 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 of staff/household records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #:	22VAC40-111-70-A
Description:	Based on a review of records, one child's record does not include written acknowledgement of all required policies and procedures. Evidence: One records does not include the address of the emergency relocation destination. (Child 1)
Plan of Correction:	I will put the new location's address on the form, and update all of the children's records for the new location. I will have all of the parents sign the new forms.
Standard #:	22VAC40-191-40-D-1-a
Description:	Based on a review of records, one record does not contain documentation of a completed Central Registry Search (CPS). Evidence: Adult A's record does not include the results of a completed CPS search result.
Plan of Correction:	I will look for the form, if I cannot find it I will submit new form to be completed.

Description:	Based on a review of records, one resident record does not include documentation of a completed CPS search result. Evidence: One Household member (aged 14-17) does not have a completed CPS search on file. (Household member 3)
