Discussed required documentation and background checks for household members.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 02/08/2021 and concluded on 02/11/2021. The provider was contacted by telephone to initiate the inspection. There were 8 children present and 2 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-111-780-B
Description: Based on record review, the provider did not review with the parent, annually, the emergency contact information to ensure the information is correct and obtain the parent's signed acknowledgment of the review.
Evidence:
1) The most recent review of the emergency information in the record for Child #2 was dated 06/05/2019.

Plan of Correction: I will ask the parents to review the information immediately.

Standard #: 22VAC40-111-830-B
Description: Based on documentation review, shelter-in-place procedures shall be practiced a minimum of twice per year.
Evidence:
1) The emergency drills log submitted to the licensing inspector by the provider, did not document any shelter-in-place drills for January-December of 2020.

Plan of Correction: The provider shall ensure to incorporate a minimum of two shelter-in-place drills annually.

Standard #: 22VAC40-111-180-A
Description: Based on record review, the provider did not obtain for each caregiver a current Report of Tuberculosis(Tb) Screening form, every two years.
Evidence:
1) The most recent Tb screening form in the record for Staff #2 was dated 05/22/2018.

Plan of Correction: Staff #2 has been cautious with COVID and has not wanted to go out and get it done.

Standard #: 22VAC40-111-210-A
Description: Based on record review and interview, caregivers did not obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.
Evidence:
1) The records for Staff #1 and #2 did not contain documentation of having completed training for the year 2020.
2) The provider stated that they did not complete any training for the year 2020.

Plan of Correction: Moving forward, caregivers shall complete the required training hours.

Standard #: 22VAC40-191-40-D-1-a
Description: Based on record review and interview, it was determined that the center did not ensure that Central Registry search results are obtained every five years.
Evidence:
1) On the date of inspection, a current Central Registry search result was not available for review. The most recent Central Registry search request result available in the record for Staff #1 was dated 09/03/2015. A new Central Registry search request result was due on 09/04/2020.
2) The provider stated that she requested the new search but cannot find the results.

Plan of Correction: The provider shall locate the search results and include in the record. If it cannot be located within 10 days, a new search shall be requested.
