Evidence:
Household resident C turned 18 years of age more than 30 days ago. The provider has not asked this household resident to sign a Sworn Disclosure Statement.Plan of Correction: I will have her sign the form today, and send a copy to my Inspector.
Standard #: 63.2-1721.1-B-2 Description: Based on a review of background checks, it was determined that one adult household resident has not submitted to a fingerprint background check.
Evidence:
Household resident C turned 18 years of age more than 30 days ago. This person has not completed a fingerprint background check.Plan of Correction: She will make her appointment this week. I will send the results to my Inspector.
