Corrections to background check violations must be initiated within 10 days from the date of this inspection.

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated and concluded on 2/16/2021. The provider was contacted by telephone to initiate the inspection. There were 0 children present. The inspector reviewed with the provider a list of items required to complete the inspection. The Inspector reviewed 3 provider/adult household member records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #: 22VAC40-191-40-D-4-a Description: Based on a review of background checks, it was determined that the provider did not obtain a signed Sworn Disclosure Statement for one household resident within 30 days of that person turning 18 years of age.
Evidence:
Household resident C turned 18 years of age more than 30 days ago. The provider has not asked this household resident to sign a Sworn Disclosure Statement.Plan of Correction: I will have her sign the form today, and send a copy to my Inspector.
Standard #: 63.2-1721.1-B-2 Description: Based on a review of background checks, it was determined that one adult household resident has not submitted to a fingerprint background check.
Evidence:
Household resident C turned 18 years of age more than 30 days ago. This person has not completed a fingerprint background check.Plan of Correction: She will make her appointment this week. I will send the results to my Inspector.
