This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on February 16, 2021 and concluded on February 16, 2021. The provider was contacted by telephone to initiate the inspection. There were three children present (8pts) and one caregiver. The inspector emailed the provider a list of items required to complete the inspection.
The Inspector reviewed two children?s records and one household member record and one provider record submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #: 22VAC40-111-70-A Description: Based on record review and interview, parents had not been provided in writing the provisions of the family day home's emergency preparedness and response plan.
Evidence:
1. Child #1's (First date of attendance-09/14/20) record did not contain the provisions of the Emergency Plan.
2. The provider stated that the parents had not received the Provisions of the Emergency Plan form.Plan of Correction: The provider will give the parents the Provisions of the Emergency Plan form and have the parents to sign it.
