This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 01/11/2021 and concluded on 02/12/2021. The provider was contacted by email to initiate the inspection. There were five children present and two staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed two children's records, one household record, and one staff record submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #: 22VAC40-111-120-B Description: Based on record review and discussion with the provider, the provider failed to ensure all assistant/substitute provider record contained all elements as required by the standards.
Evidence:

Standard #: 22VAC40-111-90-A Description: Based on record review, the provider did not obtain immunization records for each enrolled child prior to their first day of attendance.
