Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

An unannounced monitoring inspection was initiated on 2/09/21, and concluded on 2/24/21, approximately from 2:15 pm to 3:30 pm. There were 9 children, 3 were providers children (total of 18 points) supervised by 3 direct care staff. The inspector emailed the provider a list of items required to complete the inspection.

Violations were observed during the inspection.

The physical plant, 3 staff record, 1 household record, 2 children?s record, children activities, children's activities, emergency procedures, drills and supplies were inspected. During the inspection, children were resting. The interactions between the children and caregivers were seen to be positive and constructive in nature.

Violations:
Standard #: 22VAC40-111-780-B
Description: Based on children's records review, the provider did not maintain annual review with the parent emergency contact information to ensure information is correct.
Evidence:Child # 2 (date of enrollment 1/02/2020), did not have annual review documented in record when reviewed.

Plan of Correction: Dad signed 1 year review at pickup today 2/24/2021

Standard #: 22VAC40-111-180-A
Description: Based on staff records review, the provider did not maintain that each caregiver had a current Report of Tuberculosis Screening form every two years from the date of the first screening.
Evidence: Staff #3 did not have a current Report of Tuberculosis Screening form every two years from the date of the first screening. Staff #3 last screening was on 1/15/2019.

Plan of Correction: My employee has an appointment for saturday and will have the results to me on Monday
