Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A subsidy health and safety inspection was initiated on 2/25/2021 and concluded on 2/26/2021. The provider was contacted by telephone to initiate the inspection. There were two children present with the provider. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed two children?s records and the provider?s and assistants record submitted by the provider to ensure documentation was complete.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-111-100-A
Description: Based on record review and interview, the provider failed to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Evidence:

1. The record for child 1 documents the start date as 3/25/19. The record does not contain documentation of a physical.
2. The record for child 2 documents the start date as 10/13/20. The record does not contain documentation of a physical.
3. The record for child 4 documents the start date as 2/3/20. The record does not contain documentation of a physical.

Plan of Correction: The parents of child 1, child 2 and child 3 will be advised to provide documentation of physical examination by 3/16/21 or their children cannot return.

Standard #: 22VAC40-111-90-A
Description: Based on record review and interview, the provider failed to obtain documentation that the child has been adequately immunized before a child may attend the family day home.

Evidence:

1. Child 1's start date was documented as 3/25/19. The record did not contain documentation of immunizations.
2. Child 2's start date was documented as 10/13/20. The record did not contain documentation of immunizations.
3. The director verified she does not have immunization records for child 1 and child 2.

Plan of Correction: The parents of child 1 and child 2 will be advised to provide immunization records by 3/16/21 or their children cannot return.
