54.1 Provider must be MAT certified to administer prescription medication.

Technical Assistance:
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Model forms for emergency procedures, staff orientation training and staff information were sent via email to the provider.

Discussed annual review requirements which will be due within the next 6 months.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was conducted via Facetime with the provider. There were no children in care today. Observation of the physical plant was made virtually. One child currently enrolled at time. One child record and one staff record were reviewed today. No changes to household members from the last inspection. Provider demonstrates a good knowledge of the standards during the six month Conditional period. Thank you for your cooperation during this inspection process. Description: Based on staff record review, one staff record did not contain all required information.
Evidence: Staff #2's record did not contain an emergency contact and date of employment.

Plan of Correction: This information will be obtained and kept in the staff record.

Standard #: 22VAC40-111-230-A
Description: Based on staff record review, written documentation of training for one staff was not maintained.
Evidence: Written documentation of orientation training for Staff #2 was not maintained.

Plan of Correction: Staff will sign the orientation training document and it will be maintained in their record.
