- Technical Assistance:
The Licensing Inspector has reviewed with the Provider COVID-19 Essential Guidance for Child Care programs.
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated and concluded on 03/04/2021. The Provider was contacted by telephone to initiate the inspection. There were (10) children present, (25) points, with (2) caregivers. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed (2) children?s records, (1) staff record, and (1) household member's record submitted by the Family Day Home (FDH) to ensure documentation was complete.
Information gathered during the inspection determined (2) non-compliances with applicable standards or law and were documented on the violation notice issued to the FDH.

Standard #: 22VAC40-111-100-A Description: Based on review, 1 of 2 files reviewed indicated that the Provider did not obtain documentation of a physical examination (by or under the direction of a physician) within 30 days after the child's first day of attendance.
1. On the date of inspection (03/04/2021), Child #1 did not have documentation of a physical examination on file.
2. The start date for Child #1 was listed as 01/04/2021 and was in attendance on the date of inspection.
Plan of Correction: The parent of Child #1 shall be contacted, documentation obtained, and placed on file.
Standard #: 22VAC40-111-170-A Description: Based on review, 1 of 2 files indicated that the Provider did not obtain for each adult household member prior to coming into contact with children a current Report of Tuberculosis (TB) Screening.
1. On the date of inspection, Household Member #3 did not have documentation of a TB screening on file at the Family Day Home.
2. Household Member #3 has recently became of adult age.
Plan of Correction: This documentation shall be obtained and placed on file.
