Technical Assistance:
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

State model form for evacuation drills was sent to the provider as a resource.
State model form for references was sent to the provider as a resource.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was conducted via Facetime with the provider. A total of 9 children(24pts) with 2 staff were present. Observation of the program area, crib room, nap room, food preparation area was made. The children were observed sitting on the carpet waiting to transition to lunch. Lunch today was chicken, lentil rice, yogurt and berries. No medication needs or food allergies were identified. A review of 3 children's records and 1 new staff record was made. Standard #: 22VAC40-111-830-C
Description: Based on review of the provider's documented emergency evacuation drills, not all required documentation was found.
Evidence:
The evacuation drill documentation did not include the identity of the person conducting the drill, the method of notification of the drill, the number of caregivers and children participating, the weather conditions and the length of time it took to complete each drill.

Plan of Correction: We will use the state model form or revise our form to include all this required information.

Standard #: 22VAC40-111-120-B
Description: Based on review of staff records, one record did not contain all the required information.
Staff #2's record did not include age verification and two complete references. The references reviewed did not include the date the references were received.

Plan of Correction: We will ensure going forward that all references are obtaining in a way that verifies the references was obtained correctly.

Standard #: 22VAC40-111-80-C
Description: Based on review of the proof of identity for one child, documentation was incomplete.
Evidence: Child #5's proof of identity did not include the method of verification of the child's age and identity. The certificate number was not included in the verification documentation.

Plan of Correction: We will obtain this information and document the number on the proof of identity.

Standard #: 22VAC40-111-480-D
Description: Based on observation of the programming area, one piece of equipment was not used in accordance with manufacturer's instructions.
Evidence:
Child #1 (age 2 years) was observed sitting in a bouncy seat. The recommended age for this piece of equipment is from newborn to six months of age.

Plan of Correction: We will ensure that children older than six months will not use this equipment.

Standard #: 63.2-1720.1-B-4
Description: Based on review of staff records, background checks for 1 staff was not obtained.
Evidence:
Staff #2 was hired in 8/2020. An out of state sex offender search and a Central Registry search from NJ has not been requested. The sex offender search must be completed prior to hire and the Central Registry search results must be obtained within 45 days of hire.

Plan of Correction: The sex offender search will be completed today. The central registry search will be requested from NJ today. The results of both searches will be forwarded to the licensing inspector for review.
