This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A subsidy health and safety inspection was conducted on 3/2/21. The Provider was contacted by Facetime to initiate the inspection. There were 9 children(24pts) present and 2 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.

Standard #: 22VAC40-665-190-A Description: Based on review of staff records, one TB screening was not obtained prior to hire.
Evidence: Staff #2 was hired in August 2020, the documentation for their TB screening is dated 12/20. Subsidy requires that TB screenings are obtained prior to hire.Plan of Correction: We will ensure that TB screenings are obtained prior to hire but are no older than 30 days from the hire date.
Standard #: 22VAC40-665-230-B Description: Based on review of staff records, required training was not completed.
Evidence: Staff #2 was hired in August 2020, the Preservice training has not been completed.Plan of Correction: This staff will complete this training.
Standard #: 22VAC40-665-230-E-2 Description: Based on review of staff records, First Aid certification was not obtained for 1 staff.
Evidence: Staff #2 does not have current First Aid certification.Plan of Correction: This staff will complete an approved first aid course.
