Technical Assistance:

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. Consultation was given to contact OBI for background check follow-up.

Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 3/10/2021 and concluded on 03/22/2021. The Provider was contacted by email to initiate the inspection. There were six children with onecaregivers for 8 points which is within ratio for supervision. Reviewed children and household member?s records, trainings and emergency preparation. Food is bought from home. No medication was reported to be on-site the day of the inspection. Younger children were sleeping and the older three were doing quiet activities. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. Description:	Based on record review and caregiver statment, a household member does not have a current record of tuberculosos within 2 years of the previous record. Evidence: HHM #1's TB expired 8/2020 and a new test or screening has not been obtained as of 3/22/2021.

Description:	Based on record review and caregiver statement, 16 hours of annual training has not been obtained per caregiver. Evidence: Both the provider and assistant completed 14 hours of the 16 annual hours required for 2020.

Description:	Based on record review and provider statement, a household member has not repeated a Central Registry check within 5 years of the previous record. Evidence: HHM #3's Central Registry expired 2/2020 and a new completed check was not on-site. Caregiver states that the new one was returned as it was missing information but a follow up call or email was not sent.
