Technical Assistance:
Technical assistance on staff and household records.

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

An unannounced monitoring inspection was initiated on 3/09/2021, and concluded on 3/18/2021, approximately from 10:15 a.m. to 11:30 a.m.. There were 12 children supervised by 2 direct care staff. The inspector emailed the provider a list of items required to complete the inspection.

The physical plant, 2 staff records, 4 children?s records, 2 household records, children activities, children's activities, emergency procedures, drills and supplies were inspected. During the inspection, children were observed in group and individual activities. The interactions between the children and caregivers were seen to be positive and constructive in nature.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Description: Based on staff records review, the provider did not maintain for each caregiver and adult household member a current Report of Tuberculosis Screening form every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.
Staff #1 Tuberculosis Screening expired on June 20, 2018.
Household member #1 Tuberculosis Screening expired on 8/02/2018.
Household member #3 Tuberculosis Screening expired on 6/26/20218.

Plan of Correction: Will get TB test for staff #1 and household 1 &3

Standard #: 63.2-1720.1-B-4
Description: Based on staff records review, the provider did not obtain a copy of the results a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.
Evidence: Staff #2 central registry results expired on 3/02/2016.

Plan of Correction: Please see the attached central registry results dated 12/04/2020 for staff #2. Was in the file overlooked during inspection.
