Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated and concluded on 4/07/2021. The provider was contacted by telephone to initiate the inspection. There were (5) children present and (2) caregivers. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed (2) children?s records and (2) caregiver records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-111-830-B
Description: Based on interview, the provider did not practice shelter in place procedures a minimum of twice per year.
Evidence:
1. The provider stated they only practiced the shelter in place procedure once in 2020.

Plan of Correction: I will make sure I get two done this year.

Standard #: 22VAC40-111-180-A
Description: Based on review and interview, the provider did not obtain updated tuberculosis tests or screenings for each caregiver every two years.
Evidence:
1. Caregiver #2 had a TB test or screening dated 2/14/2019. The provider stated the caregiver had an appointment scheduled for 4/22/2021.

Plan of Correction: An appointment is scheduled for April 22, 2021.
