Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated and concluded on April 21, 2021. The provider was contacted by telephone to initiate the inspection. There were 7 children(24pts) present and two staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed two children?s records, one household member's record and two staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-111-780-B
Description: Based on record review, there is no documentation of annual review of emergency contact information.

Evidence:
1. Child #1's(start date-11/11/19) record did not have documentation of an annual review of emergency contact information.

Plan of Correction: The provider will have the parents to review the emergency contact information and document the record.

Standard #: 22VAC40-111-800-B
Description: Based on record review and interview, the provider did not review the emergency plan annually.

Evidence:
1. The emergency plan was last reviewed on 11/15/2019. The provider confirmed that the plan was reviewed and documented last on 11/15/2019.

Plan of Correction: The provider will review and document the emergency plan.

Standard #: 22VAC40-111-210-A
Description: Based on record review and interview, caregivers did not obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.

Evidence:
1. Staff #1 and Staff #2 did not obtain any annual training. Staff #1 confirmed that annual training was not obtained for the year.

Plan of Correction: The staff members will take 16 hours of training and send to the licensing inspector.

Standard #: 22VAC40-111-350-E
Description: Based on observation and interview, provider did not maintain water tap used by children within a range of 105?F to 120?F.

Evidence:
1. The tap that children are using to wash their hands is at room temperature. The water sits in a container for the entire time.
2. The provider confirmed that children use this water to wash their hands and that the water is only at room temperature.

Plan of Correction: The provider will wash the children's hands at a sink where there is running water and within the required range.
