Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on April 29, 2021 and concluded on April 30, 2021. The provider was contacted by telephone to initiate the inspection. There were 12 children present and 2 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-111-830-A
Description: Based on record review, the provider did not practice monnthly evacuation drills.

Evidence: The provider stated drills were completed from October 2020 to February 2021 but there was no documentation to support the statement.

Plan of Correction: I will make sure to do practice drills and document the drills.

Standard #: 22VAC40-111-120-A
Description: Based on record review, the provider did not maintin a record for each caregiver.

Evidence: Caregiver A did not have a record. Provider stated she had not completed it. (Caregiver A, DOH 04/12/21)

Plan of Correction: I will complete the file for the caregiver.

Standard #: 22VAC40-111-340-C
Description: Based on observation, the bathroom did not contain all required elements.

Evidence: There was no papertowel observed in the bathroom. The provider stated the children use a cloth towel to dry their hands after washing hands.

Plan of Correction: At the time of inspection, the provider placed papertowel in the bathroom,

Standard #: 22VAC40-111-480-D
Description: Based on observation, equipment in the center was not utilized according to manufacturing instructions.

Evidence: Child A was observed in a high chair without the safety straps on Child A.

Plan of Correction: At the time of inspection, Child A was strapped in with the safety straps.

Standard #: 22VAC40-191-60-B
Description: Based on record review and interview, the provider did not ensure all caregivers have a completed sworn statement or affirmation by the first day of employment.

Evidence: Caregiver A was hired on April 12, 2021. The provider stated Caregiver A did not have a completed sworn statement on file.

Plan of Correction: I will make sure Caregiver A has a completed background check.

Standard #: 63.2-1720.1-B-2
Description: Based on record review and interview, all caregivers did not submit to fingerprint based background check prior to their first day of employment.

Evidence: Caregiver A was hired on April 12, 2021. At the time of inspection, the provider stated Caregiver A did not have a completed fingerprint based criminal history background check.

Plan of Correction: I will make sure to complete her background check.
