54.1 Provider must be MAT certified to administer prescription medication.

Technical Assistance:
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Consultation was given on transportation requirements (first aid kit, children's emergency forms and permission to transport as well as provider information), updated emergency preparedness plan from County form to State Form,

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

An initial inspection was initiated on 05/07/2021 once the required background checks were available for review. The Provider was contacted by email and telephone to review initial application and provide updates. There were two children with one caregivers for 2 points which is within ratio for supervision. Reviewed nutrition, children and household member?s records, trainings and emergency preparation. No medication was reported to be on-site the day of the inspection.

Description: Based on provider statement, the first aid kit was missing an item.
Evidence:
The first aid kit was missing a second triangular bandage.

Plan of Correction: Another triangular bandage was implemented in the first aid kit.

Standard #: 22VAC40-111-920
Description: Based on provider statement, a menu is not available.
Evidence:
The menu is not posted for parents with date, any substituted food or on file for one week.

Plan of Correction: The food was posted on the blackboard. It also has a date.

Standard #: 22VAC40-111-60-B
Description: Based on record review, 1 of 1 child's file is incomplete.
Evidence:
Child #1 was missing the health insurance policy number.

Plan of Correction: The health insurance number has already been entered in the file.
