Technical Assistance:
The Licensing Inspector has reviewed with the Provider COVID-19 Essential Guidance for Child Care programs.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated and concluded on 05/26/2021. The Provider was contacted by telephone to initiate the inspection. There were (8) children present, (16) points, with (2) caregivers. The inspector emailed the Provider a list of items required to complete the inspection. The Inspector reviewed (2) children?s records, (2) staff records, and (1) household member's record submitted by the Family Day Home (FDH) to ensure documentation was complete.

Information gathered during the inspection determined (1) non-compliance with applicable code and was documented on a Violation Notice issued to the FDH.

Violations:
Standard #: 63.2-1720.1-A
Description: Based on review, 1 of 2 staff files, and 1 of 1 household member's file indicated that had background checks and a sworn statement older than five years.

Evidence -
1. On the date of inspection (05/26/2021), the following individuals had a search of the central registry of Virginia (CPS check) that was older than five years: Staff #1 (CPS check dated - 02/18/2016), and Household Member #1 (CPS check dated - 02/18/2016).
2. The following person had a Sworn Statement or Affirmation (SDS) form that was older than five years: Household Member #1 (SDS dated - 01/30/2016).

Plan of Correction: All outdated documentation shall be updated. New requests for both CPS checks have already been submitted. A new SDS shall be completed by Household Member #1.
