Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 5/20/2021 and concluded on 5/20/2021. The provider was contacted by telephone to initiate the inspection. There were 10 children present and 3 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-111-120-B
Description: Based on records review, staff records were incomplete.

Evidence:
Staff #2's record is missing one of two references as to their character and reputation, as well as competency required prior to the first day of employment.

Plan of Correction: I have obtained a second reference for the staff member.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records did not all contain required information.

Evidence:
1. Child#1's record is missing the employer address and employer phone number of both parents, the address for one emergency contact and the child's medical policy number.
2. Child#2's record is missing one of two emergency contacts.

Plan of Correction: I have obtained the missing information.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, the provider did not obtain a central registry check for each staff member within 30 days of employment.

Evidence:
Staff#2's record did not include a central registry search. The staff member's first day of employment was documented as 2/16/2021.

Plan of Correction: The staff member will not return to work until we receive the results.

Standard #: 63.2-1720.1-B-2
Description: Based on record review, the provider did not obtain a fingerprint based background check for each staff member prior to their first day of employment.

Evidence:
Staff#1's record did not include a fingerprint record check prior to their first day of employment. The staff member's first day of employment was documented as 5/10/2021.
Staff#2's record did not include a fingerprint record check prior to their first day of employment. The staff member's first day of employment was documented as 2/16/2021.

Plan of Correction: Staff#1 has received her fingerprint results. Staff#2 will not return to work until we received the results.
