Technical Assistance:
Technical assistance was offered regarding the following: Emergency drills, children and staff records, background checks, capacity and points for the family day home. Additional information regarding background checks was emailed to the provider for review and reference.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A non mandated monitoring inspection was initiated on 6/8/21 and concluded on 6/10/21. The provider was contacted by telephone to initiate the inspection. The purpose of this inspection was to assess the care of children and daycare activities during the conditional period. There were 5 children (13 points) and 2 caregivers present. During the virtual portion of the inspection, the children were observed playing indoors and eating lunch. The lunch menu included a mixed vegetable stew with rice and milk. A discussion was held about maintaining the approved capacity and points for the family day home when accepting part time children. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 1 caregiver's record submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. Standard #: 22VAC40-111-120-B
Description: Based on staff interview, the provider did not obtain documentation of two reference checks for an employee before the first day of employment.
Evidence:
1. Caregiver # 1's first day of employment was 5/24/21.
2. The provider stated that she had not completed two reference checks for Caregiver # 1.

Plan of Correction: The reference checks will be completed and the inspector will be notified.

Standard #: 22VAC40-111-60-A
Description: Based on staff interview, the provider did not maintain an up to date record each child in care.
Evidence:
1. The provider stated that Child # 1's first day of enrollment was 6/4/21.
2. The provider stated that she did not obtain any paperwork for Child # 1. No documents were available to review for Child # 1 during the inspection.

Plan of Correction: I am in the process of obtaining paperwork for Child # 1.

Standard #: 22VAC40-111-90-A
Description: Based on staff interview, the provider did not obtain immunization records for a child in care before the first day of enrollment.
Evidence:
1. Child# 1's first day of enrollment was 6/4/21.
2. The provider stated that she did not obtain the immunization records for Child # 1.

Plan of Correction: I have asked the parents to submit these records.

Standard #: 22VAC40-191-60-B
Description: Based on staff interview, the provider did not obtain the Sworn Disclosure Statement (SDS) for an employee prior to the first day of employment.
Evidence:
1. Caregiver # 1's first day of employment was 5/24/21.
2. The provider stated on 6/10/21 that Caregiver # 1 had not completed the SDS prior to employment, and will have it completed this day.

Plan of Correction: The employee will complete the form today. A copy will be sent to the inspector.

Standard #: 63.2-1720.1-B-2
Description: Based on records review, the provider did not obtain results of the fingerprint based criminal history check for an employee before the first day of employment.
Evidence:
1. Caregiver # 1's record indicated that her first day of employment was 5/24/21.
2. Caregiver # 1's fingerprint results were obtained on 6/7/21.

Plan of Correction: I will get these results before I hire a new employee.
