Evidence: Child 2 was observed laying in a crib while drinking a botte. The provider stated they put the child in the crib with the bottle.

Plan of Correction: The child was taken out of the crib at the time of inspection.

Description: Based on observation, hazardous materials were accessible to children.

Evidence: A cleaning solution was observed on the back of the toilet accessible to childreh utilizing the bathroom.

Plan of Correction: The cleaning product was placed in a locked cabinet in the bathroom.

Standard #: 22VAC40-111-530-B
Description: Based on observation, there was not a tight-fitting bottom cover for a crib mattress.

Evidence: A child was observed in a crib that did not have a tight-fitting bottom cover.

Plan of Correction: We will get a new cover for the mattress.
