Technical Assistance:

One or more background checks were missing or incomplete during today's inspection. The licensee must provide documentation that the missing background check has been initiated within 10 business days to the licensing inspector. Failure to do so could potentially result in enforcement action.

Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated on 6/21/2021 and concluded on 6/25/2021. The Provider was contacted by telephone to initiate the inspection. There were 6 children present with 2 staff. The inspector emailed the Provider a list of items required to complete the inspection. The inspector reviewed 2 children?s records, 2 staff records and 3 household member records submitted by the Provider to ensure documentation was complete. The information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the facility. The licensing inspector has reviewed with the Provider COVID-19 Essential Guidance for Child Care programs.

Description:	Based on record review and interview, children's records were not maintained up-to-date. Evidence: Child #2's record not reviewed annually. The most recent documented review of Child #2's record is dated 6/8/2020.

Standard #:	22VAC40-111-690-D
Description:	Based on interview, diaper changing surfaces were not cleaned and sanitized after each use. Evidence: Caregiver #1 stated that after each diaper change they use a disinfectant wipe to clean the changing mat. Cleaning and sanitizing should be a two-step process.

Description:	Based on record review and interview, the provider has not obtained a repeat central registry background check for each household member every five years. Evidence: Household member #3's most recent central registry background check is dated 2/5/2015. Caregiver #1 stated that the background check was notarized and put in the mail but the results didn't come. Caregiver #1 confirmed that they have not followed up with the Office of Background Investigations to inquire about the whereabouts of the background check results.
