This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).
A monitoring inspection was initiated on July 26, 2021 and concluded on July 28, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 2 children present, ranging in ages from birth to 12 years, with 1 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 4 household members records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22VAC40-111-180-A Description: Based on review of four household member records, one does not contain the required documentation.
Evidence:
The record for household member number one does not contain a current report of tuberculosis screening form. The current form expired in April 2021.Plan of Correction: The tuberculosis screening form will be completed and placed in household members record. I will ensure all screenings are completed before they expire.
