Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A renewal inspection was initiated and concluded on August 6, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were four children(10pts) present, ranging in ages from 8 months to 3 years old, with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of two child records and 2 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-111-800-B
Description: Based on record review and interview, the provider did not review and document in writing the annual review of the emergency plan.
Evidence:
1. The provider did not document in writing the annual review of the emergency plan. The last date for documentation of the annual review was 01/11/2019.
2. The provider confirmed that the information for the emergency plan is the same, but written documentation was not completed since 01/11/2019.

Plan of Correction: The provider will review and document in writing that the emergency plan has been reviewed for the year 2021.

Standard #: 22VAC40-111-180-A
Description: Based on record review and interview, the provider did not obtain for each caregiver and household member a current Report of Tuberculosis Screening Form every two years from the date screening.

Evidence:
1. Caregiver #1's, Caregiver #2's, and Household Members #1's record did not contain documentation of a current Tuberculosis Screening. Caregiver #1's last screening was dated 07/25/2018. Caregiver #2's last screening was dated 07/10/2019, Household Member #1's last screening was dated 07/10/2019.
2. Caregiver #1 stated that Tuberculosis(TB) screenings were expired and appointments have been made to get TB screenings completed.

Plan of Correction: Appointments have been secured for screenings and will be completed. Once screenings have been completed, the provider will send the licensing inspector a copy of the results.
