Technical Assistance:
This monitoring inspection was part of a joint inspection conducted with a review of applicable subsidy regulations. Please see the inspection reports for the subsidy inspection for more information.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the program.

A monitoring inspection was initiated and concluded on 07/30/2021. The Provider was contacted by telephone and a virtual inspection was conducted. There were 6 children present (16 points), ranging from 3 months to 3 years in age, with 3 caregivers supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication; special care and emergencies; and nutrition. A total of 8 children records, 2 household member records, and 3 caregiver records were reviewed.

Information gathered during the inspection determined that there was a non-compliance with an applicable standard. This violation was documented on a Violation Notice issued to the program.

Standard #: 22VAC40-111-180-A
Description: Based on review, 1 of 2 household member files indicated that the Provider did not obtain for an adult household member a current Report of Tuberculosis (TB) Screening form within the past two years.

Evidence -
1. On the date of inspection (07/30/2021), the most current TB documentation on file for Household Member #2 was dated as 11/19/2018.
2. This observation was confirmed by the Provider on the date of inspection.

Plan of Correction: Household Member #2 shall obtain updated TB documentation and place it on file at the FDH.
