Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and a virtual tour of the program.

A renewal inspection was initiated on 08/17/2021 and concluded on 08/17/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 8 children present, ranging in ages from 3 months to 4 years, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, personnel, physical environment and equipment, care of children, nutrition, emergencies, and background checks. A total of 2 child records, 2 caregiver records, and all household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-111-760-A-1
Description: Based on observation, the first aid kit did not contain all required supplies.

Evidence:
1. The first aid kit did not contain triangular bandages

Plan of Correction: I will buy them today.

Standard #: 22VAC40-111-70-B
Description: Based on record review, the provider did not obtain the parent's written acknowledgement of the receipt of all required information.

Evidence:
1. The documentation in the record for Child #1 did not contain written acknowledgement from the parents of the provider's policies for the administration of medications.

Plan of Correction: I will have the parent sign the form today.

Standard #: 22VAC40-111-80-C
Description: Based on record review, 1 of 2 children's records did not contain all required information of verification of proof of identification for the child.

Evidence:
The record for Child #1 did not contain documentation of date and person verifying original proof of identification.

Plan of Correction: We will document the information.

Standard #: 22VAC40-111-180-A
Description: Based on record review, the provider did not obtain a current Report of Tuberculosis Screening form every two years for one caregiver and one adult household member.

Evidence:
1. The most recent documentation of a tuberculosis screening in the record for Caregiver #1 was dated 7/9/2019
2. The most recent documentation of a tuberculosis screening in the record for Household Member #1 was dated 1/30/2018.

Plan of Correction: We will make an appointment to get our tuberculosis screening.

Standard #: 22VAC40-111-460-D
Description: Based on record review, the provider did not maintain documentation of a current rabies vaccination.

Evidence:
1. Documentation maintained by the provider of the rabies vaccination for the provider's pet was dated 1/30/2018 and expired on 1/30/2021.

Plan of Correction: I will contact the veterinarian's office and ask for documentation of the vaccinations.
