Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and interviews. A renewal inspection was initiated on 8/20/21 and concluded on 8/26/21. The provider was contacted by telephone and a virtual inspection was conducted. There were four children present, all age two, with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records and three staff and household member?s records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #:	63.2-1721.1-A
Description:	Based on record review and interview, the provider failed to obtain an updated central registry background check five years since the previous central registry check for all providers and household members ages 14 and older. Evidence: 1. The most current central registry check for the provider is dated 7/28/16. 2. The most current central registry check for household member 2 is dated 3/23/16.
Plan of Correction:	Updated central registry checks for the provider and household member 2 was mailed 8/6/21. In the future the provider will keep track of when background checks need to be updated.
Standard #:	63.2-1721.1-B-2
Description:	Based on record review and interview, the provider failed to obtain a fingerprint background check for a household member that turned 18 on 4/18/20. Evidence: The provider stated she did not obtain a fingerprint background check for household member 2. The provider verified the date of birth for household member 2.
Plan of Correction:	A fingerprint background check will be scheduled for Mikeya. In the future the provider will ensure all requirements for background checks will be followed as required.
