Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and possible time table to conduct monitoring inspections on site and in person.

Comments:
A monitoring inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the program.

This inspection was initiated and concluded on 10/07/2021. The Provider was contacted and a virtual inspection was conducted. There were 8 children present (19 points), ranging from 12 months to 4 years in age, with 2 caregivers supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication; special care and emergencies; and nutrition. A total of 4 children records, 3 household records, and 2 staff records were reviewed.

Information gathered during the inspection determined that there was a non-compliance with an applicable standard. This violation was documented on the Violation Notice issued to the program.

Violations:
Standard #: 22VAC40-111-180-A
Description: Based on review, 2 of 2 staff files, and 2 of 3 household records indicated that the Provider did not obtain (for each caregiver and adult household member) a current Report of Tuberculosis Screening (TB) form, in accordance with the requirements in 22 VAC 40-111-170, every two years.

Evidence -
1. On the date of inspection (10/07/2021) the following staff files had TB documentation older than two years: Staff #1 (TB documentation dated - 05/17/2019), Staff #2 (TB documentation dated - 05/17/2019).
2. On the date of inspection, the following household files had TB documentation older than two years: Household Member #1 (TB documentation dated - 06/06/2019), Household Member #2 (TB documentation dated - 05/14/2019).

Plan of Correction: All applicable staff and household members shall obtain updated TB documentation. This documentation shall be placed on file at the Family Day Home.
