Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone/email contacts, documents review, interviews and a tour of the program. A renewal inspection was initiated on 10/6 and concluded on 10/7. The provider was contacted by email and a virtual inspection was conducted. There were 9 children present, ranging in ages from 1 to 4 years, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 22VAC40-111-60-B
Description: Based on file review, 1 out of 2 children's records did not have all required information.
Evidence
1. Child #1 did not have the policy number for their health insurance.

Plan of Correction: This will be corrected.

Standard #: 22VAC40-111-210-A
Description: Based on file review, 2 caregivers did not provide documentation of having attended a minimum of 16 hours of annual training.
Evidence
1. Caregiver #1 showed documentation of having received 10 hours of annual training.
2. Caregiver #2 showed documentation of having received 13 hours of annual training.

Plan of Correction: Both caregivers will attend at least 16 hours of training within the next year.
